Saint Anthony's Home Health Care provides care to the ill or convalescing person, aimed at optimizing independent functioning.
• Enables early discharge or prevents hospitalization; and
• Enables the chronically ill or aged person to remain in their own home as long as possible.The Home Health Care Department provides quality multidisciplinary health care in the home and fosters the integrity of family life by helping each member to strive toward an optimal state of spiritual, mental, social and physical well-being. Care is consistent with the total needs of the individual and family, within the recommendations of the patient's physician and treatment plan.
To be eligible for Home Health Care, the following requirements are necessary:
- The patient's physician must order the needed services.
- Most patients should be homebound and in need of the type of services offered. Homebound status is dependent on insurance provisions.
- The home should be appropriate for proper care of the patient's needs.

How to Arrange for Home Health Services
Services may be arranged by contacting your physician, case manager, or ask a family member to inquire for you. Services must be approved by the physician.
When a patient is hospitalized, the Social Work or discharge planning department of the hospital will usually arrange Home Health Care. If you desire Home Health Care following discharge from the hospital, contact our Social Work department at 618/465-4522.
You do not need to be hospitalized to receive Home Health Care. Anyone who feels they can benefit from these services should call Saint Anthony’s Home Health office at 618/463-5683. Our referral coordinator can answer questions and instruct you on contacting your physician.
