Co-pays, if required, and payment in full of any outstanding balance on your account which has been identified as your responsibility, are expected at the time of service. There could be a charge for broken or missed appointments without 24 hour notice, for copies of medical records for any non-physician recipient or for returned checks. If you cannot provide current insurance information, you will be required to pay in full for that day’s services. If you qualify for financial assistance, a payment plan may be implemented at the discretion of the physician and the business office. Otherwise, your office visit may have to be re-scheduled.
